VIOLATIONS: Failed to submit annual operation report for two injection wells. For one injection well failed to submit an annual report for the period October 2001 through September 2002. Failed to submit a report for the period January through December 2002 for the other injection well.
RELIEF: Submit required annual operation report for each of the two injection wells within
30 days after effective date of the order.
