EMPLOYEE WAS CLIMBING A LADDER TO A WORK PLATFORM AND WAS ABOUT 6' FROM BASE WHEN HE LOST HIS BALANCE AND FELL, STRIKING HIS LEFT SHOULDER ON THE FLOOR. EMPLOYEE WAS NOT CARRYING ANYTHING UP THE LADDER AND THE LADDER WAS LTIED OFF AT THE TOP AND BOTTOM.
