USING A CHEATER PIPE ON A WRENCH TO BREAK 7/8"" MOUNTING BOLTS LOOSE. BOLT BROKE LOOSE AND PIPE SLIPPED CAUSING EMPLOYEE TO SLIP AND TWIST ANKLE.
