Narrative

Employee was roof bolting & putting in 6' bolts. While installing a 6' bolt, employee was pushing up the bolt with bolter head & while putting wrench on bolt, wrench hit the bolt & bolt slipped off wrench. Employee had left thumb on plate. Wrench slipped pinching thumb between plate & roof.
