## Narrative

Employee's crew was changing the spring suspension on a coarse refuse screen. They had hooked a chain hoist to a welded on lifting eye. The lifting eye broke as employee placed the spring into position. The spring became a projectile, striking him in the abdomen. Lost time injury.
