## Narrative

Employee was hurriedly walking to shut off blower & 12 Dryer.  He slipped and fell. Notified supervisor, but did not seek medical attention.   Contacted HR on 03/12/07 that his elbow still hurt & wanted to see Dr.   First treatment date was 03/14/07.
