## Narrative

The employee was handling scaffolding and smashed his left hand ring finger. He only received first aid at the time. During his time off, he sought medical care beyond first aid without informing the ee until he wanted to return to work. Received a full duty release on 8/31/06. Scheduled to return on 9/9/06.
