## Narrative

Employee was in the process of bending over to pick up a victaulic pipe clamp when mid back muscles spasmed on left side causing immediate pain. Employee was seen by site medic and referred to the ER where they were diagnosed with a mid-thoracic back muscle spasm, restrictions were issued.
