## Narrative

Employee was lifting a box (approximately 50lbs) to store it on a shelf approximately three feet from the ground. As the employee was lifting the box it came in contact with the shelf disrupting the movement. The employee immediately felt shooting pain in left shoulder.  It was later determined that the employee had torn rotator cuff and had surgery November 2nd, 2021.
