## Narrative

The Employee was bolting on the #2 section in the #5 Left entry. The employee was swinging the drill station in to drill the inside bolt when a rock 3'W X 3.5' Long X 4"" thick fell striking left heel Causing a laceration from the heel to the middle of foot. The entry was approximately 6 foot high and damp.
