Narrative

Employee had put the ""y"" back together after cleaning it. When restarted the flow wasn't strong, EE walked back over to check the ""y"" and when EE bent down over it the fitting busted apart and sprayed silicate on EE's face and chest with some running down into EE's eyes. Injury was not the result of a training or compliance issue.
