Narrative

IE was drilling for a PUR job in the belt entry. As EE was lowering the drill, it bumped against the rib screen, where a belt chain had been left looped through an opening. When the drill bumped the screen, the chain fell, striking IE in the side of the head as EE turned to try to avoid it. Injury was not the result of a training or compliance issue.
