Narrative

Afternoon of August 20th, employee came to our office and stated that they developed a small umbilical hernia approx. 6 mo. ago. It did not bother them so they did not notify anyone. EE said that last week the hernia became larger and started protruding. EE went to a doctor who told them to see a surgeon. EE could not provide us with details of the doctors visit until Aug. 20th.
