## Narrative

Employees were loading a 6-inch DR11 pipe into a fusion welder in the Stage 6 laydown area when the pipe became caught on the welder clamp. The employee reached in to reposition the pipe. When the pipe moved, the pressure released, causing the employee's hand to become pinched between the two sections of pipe.
