Narrative

Nuts were stuck on tramp cylinder tried multiple ways to loosen them was advised not to cut the nuts off and was directed to use a 1"" impact which resulted in the stem unscrewing from the cylinder causing a nitro discharge from accumulator shooting stem and impact into employees chest and face
