Narrative

Employee was laying down using a 5"" vacuum hose to clean underneath of a conveyor belt. The hose lost its suction and jumped forward and then back into the face of the employee. When this happened the employee got a loose tooth from it. The employee went to the dentist two weeks later and got the tooth removed.
