## Narrative

Employee was installing an incline conveyor.  He was using two chain falls and four come-a-longs. One of the come-a-longs was removed, causing the others to become overloaded, and the conveyor fell. The employee suffered broken bones in his legs, feet, spine, and pelvis area.
