## Narrative

On April 30, at 9:15 a.m., an employee fell from a ladder while dismantling a scaffold inside a tank.  The fall was less than 20 feet. The dimensions of the scaffold were 10x10x33. The employee fractured both heels and sustained a small skull fracture that required stitches. The employee was hospitalized.
