## Narrative

At approximately 8 a.m. on 05/13/15, an employee calibrating the blade pitch in a primary air fan. The fan was tagged out at the time. She rotated the fan blades by hand while wearing gloves. As she pulled down on a blade with her right hand, the blade caught the glove or fingertip of her left hand, which was resting on the flange. Her left pinky got pinched between the fan blade and fan housing, amputating the finger about 1 cm from the tip.
