## Narrative

An employee was troubleshooting on one of the lines when he decided to place an AVIS strap around an idler roll to reduce its speed. His left index finger became entangled in the AVIS strap, resulting in an amputation to his left index fingertip and bone, which was severed at the middle phalanx.
