## Narrative

An employee was making sure that no one was in the way of the forklift as the forklift driver was lowering a 20 ft long metal beam onto the ground. The employee then pulled the hand grinder's electrical cord that was on the ground so that it wouldn't get crushed by the metal beam, but instead his right index finger was crushed by the beam resulting in the amputation of his right index finger from the second knuckle.
