## Narrative

On July 3, 2015, at approximately 4:50 a.m., an employee was injured while operating a battery-operated fork tugger industrial truck. The employee picked up a load of VVTs and was backing up when the stand, which had parts on top, struck the corner of a set of steps. This shoved the stand forward, causing the employee's finger to become crushed between the handrail and the stand. The employee's right index fingertip was amputated.
