## Narrative

An employee was changing a shear's die using a hoist. The die became lodged by a piece of dowel. As the employee reached to check where it was stuck, the die dislodged while still under the tension of the hoist, causing the die to move upward and pinch his middle and ring fingers. Approximately 1/8" of the employee's left middle finger was scraped.
