## Narrative

On 8/5/15, at 7:30 a.m., a new employee received an employer-required vaccination for measles/mumps/rubella. Approximately 30 to 45 minutes after the injection, she experienced an allergic reaction; her neck swelled.  She was hospitalized and treated.
