## Narrative

On August 9, 2015, at 11:30 pm, an employee was climbing a 5 foot fixed ladder attached to the reprocessing line when the employee slipped and fell to the concrete floor.  The employee suffered two broken bones in the lower right leg and was hospitalized for surgery.
