## Narrative

At 10:30 a.m. on the production floor, an employee attempted to unjam one of the pads on the PAD machine. The blade on the machine amputated her left-hand index finger from the bed of the fingernail to the tip. She was hospitalized. The machine was still running at the time of the incident.
