## Narrative

An employee was going up a ladder to do a pre job assessment of the air conditioning system.  When he was about 4 feet from the ground, the employee's foot slipped and he lost his balance.  He fell, hitting a movable conveyor on the way to the ground. He hurt his back, head and spine.
