## Narrative

On 10/19/2015 at or around 11:15 a.m. a temporary employee was struck by a garbage truck and pinned between a utility pole and the rear bumper of the truck. The employee then fell off the rear of the truck. The employee was hospitalized with a fractured pelvis.
