## Narrative

An employee was adjusting a 100-ton jack so that it could push pins that held a gantry tube in position. When the jack was in position and pressure was starting to be applied to the pins, something slipped and caused the employee's fingers to get pinched off. About 1.5 inches of his left ring and middle fingers were amputated.
