## Narrative

An employee was inspecting a conveyer before starting production. The conveyer was in the off position at the time. The employee slipped while kneeling down to inspect the clean-out and then grabbed the side of the conveyor, which caused the clean-out cover to shut on the employee's finger, amputating the fingertip.
