## Narrative

An employee was taking electric breakers and wires out of a live electrical panel. One of the stingers from a bus bar dropped down after breaking a set screw, causing a cross phase and starting an arc flash. The employee fell to the ground and suffered burns to the eyes, face, and left forearm.
