## Narrative

An employee was attempting to remove a hose reel basket from a mat that was sitting on a trailer. When the employee went around to put a pin on the trailer to prevent the mat from falling off, the operator pushed on the load, catching the employee's fingers between the mat and pin. The employee's middle finger was amputated to the middle knuckle, and his ring finger was totally amputated.
