## Narrative

An employee was climbing up on the roof with a 10-foot extension ladder while carrying a box of air conditioner filters when the ladder shifted, causing him to fall about 6 feet to the ground. He suffered a muscle strain and bruising in his right shoulder and was hospitalized.
