## Narrative

An employee was removing a flattening plate from a coordinate measuring machine (CMM). He positioned himself on the CMM table about 38 inches off the floor and was attaching a hoist chain for a forklift boom attachment when the 2-inch probe shaft impaled his body, requiring surgical removal.
