## Narrative

An employee was pulling the release rope on a self dumping hopper. As the hopper was dumping, the hopper slid forward and the safety chain separated causing the hopper to fall off the forks of the lift truck. The employee still grasped the release rope and as the hopper fell his left ring finger was amputated between the fingernail and the first knuckle.
