## Narrative

On or about 9/7/16, at 3:45 a.m., an employee was operating an electric pallet jack with a 6' load when the employee collided with another pallet jack operator. The collision resulted in the employee's left foot being struck by/caught between the pallet jacks, partially amputating three toes.
