## Narrative

On or about September 14, 2016, an employee was working at the base of a shaft. At the same time, an overhead crane had hoisted a filled concrete bucket over the shaft. As the bucket crossed over the shaft, the bucket opened and spilled concrete onto the employee below. The employee suffered multiple fractures.
