## Narrative

On February 15, 2017, an employee was performing a leak check on a stainless steel vessel. A nut had become stripped, allowing a bolt to fly off and hit the employee in the nose and forehead. The employee was hospitalized with lacerations, a broken nose, and a broken eye socket.
