## Narrative

An employee was working on a piece of equipment and was adjusting the tube valve (approx. 30lbs) which has a manual handle and was not in the lock position. The employee put his hand inside the valve to check to see if it was flush. The lift handle released itself while the employee's finger was still in the tube resulting in the partial amputation of the third finger on the left hand.
