## Narrative

An employee was performing maintenance on a setting tool used in hydraulic fracturing. He opened a vise and turned the tool 180 degrees to make it easier to service. As he was sliding the tool back into the vise, the tool fell and caught his finger between the tool and the vise, amputating his left middle finger below the nailbed.
