## Narrative

An employee was releasing water from the hopper that was in operation. As he was releasing connections, he braced himself with his hand on an opening. His hand slipped and contacted the hopper lift resulting in amputation of the index, middle, and ring fingers up to the middle knuckles on his left hand.
