## Narrative

An employee was operating a spreader/sprayer machine to apply fertilizers and herbicides to turf. He was attempting to repair/replace a belt in the machine when his right hand was caught and pulled into its pulley system, amputating his index fingertip. The machine was not locked/tagged out at the time.
