## Narrative

On April 28, 2017, at approximately 12:14 p.m., an employee was standing on a 6-foot combination ladder to install a new service line from a utility pole to a residential home. He fell approximately 5 feet from the ladder to the ground, fracturing his coccyx bone and right wrist.
