## Narrative

On June 2, 2017, an employee was installing a ceiling joist while standing on a ladder and guardrail when the guardrail failed, causing the employee to fall approximately 12 feet. The employee suffered a spleen laceration, abdominal bleeding, and an elbow fracture, requiring hospitalization.
