## Narrative

An employee was climbing a ladder to complete a service call. A bolt holding the cable to an adjacent utility pole fell out of the pole, causing slack in the cable. As a result, the ladder bounced and the employee fell approximately 15 feet to the ground. He suffered possible fractured vertebrae.
