## Narrative

An employee was rigging down bales that were being lowered out a V door and down a slide with a winch. As the bales were being lowered to the catwalk via the slide, one section became stuck on a roller on the rig floor. When the employee reached down to free the bale, it came free and knocked him off balance. He fell through the V door and down the slide, striking an object and suffering fractured facial bones, broken ribs, and a collapsed lung.
