## Narrative

An employee was working inside a 10 to 12 feet deep excavation for the installation of a new underground pipe. He was standing inside the trench box (12 feet high x 8 feet long x 20 feet wide) when the excavation soil sheared off and hit him from behind at the calf level. The dirt pushed him forward and fractured his right ankle.
