## Narrative

A crew was offloading T-wall panels using a spreader bar and a loader. Two employees were assisting with the rigging while the loader operator positioned the loader. While the employees were connecting the second piece of rigging, the tension from the rigging attachment gave way, causing the spreader bar to strike the injured employee's hands, fracturing the left index and middle fingers and dislocating the right thumb.
