## Narrative

On August 11, 2017, at 8:30 a.m., an employee was dismounting a ladder onto a roof when the metal shingle that he stepped on detached and he fell 15 feet to the ground, fracturing his lower lumbar, cervical spine, and three ribs.  He was hospitalized.
