## Narrative

An employee was adjusting a corrugation machine's stacking section when he used a box-end wrench to loosen the bolts that hold the internal plate down. The machine then cycled, pinching his finger between the bracket (movable part) and frame (fixed part). He suffered a trans-phalangeal amputation of the right index finger. The machine was in auto mode at the time of the incident.
